4) Developing Primary Care Networks
Primary Care Networks work where there is shared passion and focus. We help PCNs unlock innovations in how primary care can meet the needs of the local population. As your critical friend we’ll help you use your scale and combined expertise to move from managing activity to meeting needs. This is an exciting new opportunity to work differently at scale.
The Work of PCNs
There are three domains of work for PCNs:
Meeting the Needs of People with Complex Problems (acuity that cant be managed at practice level)
Supporting member practices to be the best they can be (reducing unwarranted variation so that only complex needs are escalated to PCN level; and collaboration focuses on the work that is best done together; enabling robust general practice)
Collaborating with communities using an asset based approach (so that general practice only does the work that needs clinical attention)
